Registration
I, on behalf of all parties that I have registered for this workshop, authorize Manhattan Dance Project℠ (and any of its affiliates and subsidiaries) the use of their image in photos and videos for all purposes. I have read and agree to abide by all the rules and regulations outlined within 25.mdpdance.com and store.25.mdpdance.com. I also understand and acknowledge that Manhattan Dance Project℠, it’s agents, faculty and staff are not liable for personal injuries or loss of, or damage to property. Each attendee may refuse to participate in any activity and it is the attendees’ sole responsibility to determine if any condition warrants refusal to participate. If there are any doubts as to a dancer’s physical ability, please consult with your physician before participating.
Credits applied towards a future MDP℠ event are issued only in the event of an injury. In the event of an injury, MDP℠ should be notified immediately by the participant, parent, or teacher via contact@25.mdpdance.com. All credit requests must be accompanied by a physician’s note.
